Abstract The management of personal damage (at any level) arising from work is increasingly a critical issue. A fatality presents a sovereign risk to surviving family members and to people in particular roles in an organisation. Personal damage, as measured by Key Performance Indicators (KPIs) such as ‘Lost Time Injury Frequency Rate’ (LTIFR) can determine contractual relationships. As a consequence of these risks and a genuine management concern that people should not be damaged at work, the damage continues. Why is this? The problem, as always, is multi-factorial and so the solution is multi-factorial.
